The effectiveness of satellite hospital project between Viet Duc University Hospital and Dien Bien Provincial General Hospital

On October 22, the delegation of Viet Duc University Hospital led by Prof. Tran Binh Giang, MD, PhD – Director, supervised and evaluated the effectiveness of the implementation of Project 1816, Satellite Hospital project in 2020 and survey the need of training and technical transfer in 2021 of Dien Bien Provincial General Hospital (majoring in Surgery, Anesthesiology- resuscitation, and Diagnostic Imaging  …).

In the period of 2013-2020, Dien Bien Provincial General Hospital received support with 25 training packages, technology transfer to 55 trainees and 50 technical transfer respectively, acceptance supervision from Viet Duc University Hospital. At the meeting, Prof. Tran Binh Giang, MD, PhD – Director, highly appreciated the preparation of facilities, equipment and human resource of Dien Bien Provincial General Hospital to ensure the conditions when receiving the technical packages under Satellite Hospital project and Project 1816 as well as the development orientation of specialized techniques in the coming time.

Especially, with Telemedicine system of online consultation equipment, doctors at Viet Duc University Hospital have consulted emergency online with doctors of Dien Bien Provincial General Hospital, saving lives of many critical patients.

 

On March 6, 2017, for the first time, a very serious multi-traumatic emergency in Dien Bien was saved by doctors consulted through the Telemedicine system, which was the case of teacher N.T. L. (37 years old), had a motorbike-car collision, causing her child to be thrown away, while Mrs. L. was crushed by truck’s wheel on the entire chest and abdomen area. The accident caused Ms. L be crushed the abdominal wall, chest wall, rupture the diaphragm between the abdomen and thorax, causing the entire internal organs to pile up on the thorax. Ms. L was admitted to Dien Bien Provincial General Hospital in a state of severe shock, respiratory distress, shortness of breath, and her life is hanging on the edge.

 

The patient’s condition was so severe that it was impossible to transfer to Hanoi, the surgical team of Viet Duc University Hospital immediately set off. While waiting for the surgical team from Hanoi to Dien Bien, at Viet Duc University Hospital, experts continue to advice and support doctors of Dien Bien Provincial General Hospital through Telemedicine system the necessary measure to best resuscitate patient, to wait until doctors arrive then perform surgery immediately. Thereafter, the surgery lasting more than 2 hours was successful as expected despite the patient suffered from severe multiple injuries.

On July 23, 2018, a Mong ethnic patient with traumatic brain injury was taken to emergency department at Dien Bien Provincial General Hospital. The patient had to undergo cardiac stopped 3 times during the emergency. The hematoma is located in patient’s brain in different locations and is quite complicated, so doctors of Dien Bien Provincial General Hospital contacted to get support and consultation from experts of Viet Duc University Hospital. This is identified as the case of a patient in a serious, critical condition, requiring immediate management on emergency. However, in bad weather conditions, unfavorable traffic, unable to transport patient to Hanoi, the plan in which to send doctors from Viet Duc University Hospital was also considering but could not be implemented in that case. Within 30 minutes, while the patient was being admitted to the emergency operating room at Dien Bien Provincial General Hospital, in Hanoi, an emergency consultation team comprised of specialists in the field of neurosurgery and resuscitation anesthesia was convened at Online Consultation Room at Training and Direction of Healthcare Activities Center, Viet Duc University Hospital. With the Telemedicine system equipped by the Ministry of Health, Viet Duc bridgehead is ready to support and consult with expertise while doctors of Dien Bien Provincial General Hospital are directly operating the surgery. All the details of operation happening are guaranteed to be transmitted directly, continuously and most clearly from the operating room of Dien Bien Provincial General Hospital to Viet Duc University Hospital. After about 2 hours, brain hematoma was removed, the surgery ended safely and successfully, patient was promptly saved.

 

On September 4, 2020, Dien Bien Provincial General Hospital actively requested an in-depth consultation with a 14-year-old female patient who was admitted to hospital because of abdominal and chest pain after an electric bicycle – car traffic accident. Results of diagnostic imaging showed that the patient has liver and lung injuries. The patient was evaluated and advised by experts of Cardiovascular and Thoracic Surgery, Hepatobiliary Surgery of Viet Duc University Hospital for management on emergency. She was then treated and in stable health.

With Telemedicine system based on television technology integrating specialized medical equipment in remote medical examination and treatment, many critical patients in remote and critical areas were promptly saved. Prof. Tran Binh Giang, MD, PhD – Director of Viet Duc University Hospital hopes that the Telemedicine project will be implemented not only in provincial hospitals but also widely spread to district hospitals, contributing to completing development strategies of national health network to help people enjoy modern and high quality medical services at the local, reduces costs for patients due to referrals, and minimizes overcrowding with patients at national level hospitals.
