According to Suckhoedoisong.vn – Through Telemedicine system, experts of Viet Duc University Hospital conducted an online consultation with Quang Ninh Province General Hospital, successfully saved female patient N.T.T with serious heart rupture caused by traffic accident.
At 9.00 AM, January 31th, patient N.T.T, 58 years old, living in Ha Long city, Quang Ninh province had a traffic accident. After the accident, the patient was taken to Bai Chay Hospital with painful chest sign.
When admitting, she had stable hemodynamic, with signs of chest injury and broken sternum. Directed by doctors of Viet Duc University Hospital, doctors of Bai Chay Hospital have performed the CT scan for patient, it showed pericardial effusion, suspected heart rupture because the patient was starting hemodynamic disorder.
Doctors at Viet Duc University Hospital site have diagnosed the disease, planned treatment and directed surgery. Participating in the Tele-consultation were, Prof. Tran Binh Giang – Director of Viet Duc University Hospital; Ass.Prof. Nguyen Huu Uoc – Director of Cardiovascular and Thoracic Center.
Patients was assigned to emergency surgery.
The patient was taken immediately to the operating room due to tamponade with blood pressure was 60/40; The peripheral IV was placed for artificial heart-lung machine. When opening the chest by sternotomy found sternum broken and pericardium bloods.
Heart damage includes: rupture of the left atrium and right ventricle, large rupture of left pericardial connecting to the left pleura. The leading experts of Viet Duc University Hospital have instructed the surgical team from remote site to perform open-heart surgery on emergency to control bleeding and repair the lesions of atrial and ventricular as well as pericardium, remove blood clots and placed drainage in the left pleura.
With the remote support of specialists from Viet Duc University Hospital, Quang Ninh Province General Hospital doctors have successfully performed emergency surgery, saved life of critical patients after an accident.
According to Ass. Prof. Nguyen Huu Uoc – Director of Cardiovascular and Thoracic Center, Viet Duc University Hospital, this was a rare case, only central hospitals can handle it. Meanwhile, the condition of severe patients can not be transferred to specialized centers. Luckily, the patient gets timely emergency consultation through the Telemedicine system. This is especially significant during the COVID-19 epidemic.
Due to serious blood loss, the patient was transfused for nearly 5,000 ml of blood and experienced the duration of operation for more than 5 hours. Currently, the patient is stable. After surgery, the heart rate comes back normal, hemodynamic is stable, the patient is monitoring and caring in the ICU and at Quang Ninh Province General Hospital.
Prof. Tran Binh Giang emphasized that thanks to the satellite hospital project of the Ministry of Health, as a nuclear hospital of surgery, Viet Duc Hospital has organized intensive training courses for local doctors, and also, provided remote consultation in different aspects for many satellite hospitals through the Telemedicine system, connected to the operating room and consultation room of difficult cases and save lives of many patients.
