Consecutive cases of orchiectomy due to testicular torsion

According to Viet Duc University Hospital, from 9/2021 – 3/2022, there were 9 cases admitted to hospital due testicular torsion, meanwhile only preserved 2/9 cases, 7/9 cases had orchiectomy. The average age of patients admitted to hospital was 17 years old, with the youngest 14 years old and the oldest is 24. Patients with testicular torsion could not preserve testicles has many reasons like: Patients visiting too late that cause difficulty in treatment and intervention, misdiagnosed as orchitis, only come to hospital when the testicles are black and purple, unable to be preserved.

 

Especially only last 2 days from 5-6/4/2022, there were 2 patients in a row with testicular torsion come in late that had to be removed testicles. Associate Professor Nguyen Quang, Director of Andrology and Male Sexual Medicine Center informed, testicular torsion is a common andrological emergency which accounts for 17% of acute scrotal pain cases, 90% occurs in the age from 13 to 21. Testicular torsion occurs when the motile testicle rotates excessively around the spermatic cord leading to blockage of vascular system. Consequently, it led to acute vascular occlusion, if not treat in time, will lead to testicular necrosis. This is andrological emergency required urgent surgery especially in the first 6 hours.

According to Ass. Prof. Nguyen Quang, the symptoms of testicular torsion are patients often awakened in their sleep by a sharp pain in their scrotum so they tend to remember clearly the time of their pain. Clinical examination it showed the torsion side hangs higher than the normal side. Doppler ultrasound evaluation of testicular vasculature is very valuable for diagnosis of testicular torsion with typical whirlpool sign. The disease is often misdiagnosed with orchitis, which is one of the main reasons led to late diagnosis and have to remove the testicles.

 

From the these cases, doctors of Andrology and Male Sexual Medicine Center recommend, men should go to the doctor immediately when they have sudden painful in scrotum. For colleagues in all levels, when facing a patient with sudden scrotal pain, it is necessary to think of this particular disease and accurately diagnose to avoid unfortunate consequences.
