ECMO saving a 11 years old girl with severe multi-trauma

A girl patient, 11 years old, multi-traumatic shock due to a bicycle-car accident was successfully revived by doctors of Viet Duc University Hospital by extracorporeal membrane oxygenation (ECMO) technique.

 

Dr. Vu Nguyen Ha Ngan – Anesthesia and Surgical Resuscitation Center, Viet Duc University Hospital shares: N.T.P.T, a girl of 11 years old, in Phu Tho, had multi-trauma (brain trauma, bladder rupture, urethral rupture, femur and pelvic fractures), was operated on emergency for management of bladder injury and femur fracture in provincial hospital. During the surgery, patient had to have large amounts of blood transfusion, shock due to multi-trauma condition which was transferred to Viet Duc University Hospital on September 15, 2022.

 

On admission to Viet Duc University Hospital, she was a very poor respiratory condition, anemia, X-ray examination showed white lung. After one day of mechanical ventilation, the patient’s oxygen saturation did not improve, ranging from 40-50%. Because the critical situation, especially in an 11-year-old child, it requires immediate ECMO intervention to avoid the consequences of visceral hypoxia. The patient was admitted to the ICU 3 – Center for Anesthesia and Surgical Resuscitation and was examined multiple specialty to conduct ECMO within 2 hours. Doctors from the Center for Anesthesia and Surgical Resuscitation collaborated with the Cardiovascular and Thoracic Center of Viet Duc University Hospital, while actively resuscitating ventilators and placing ECMO on the patient. After 1 hour of ECMO, the patient’s oxygen saturation improved, she was taken to the Intensive Care Unit I for further treatment. After 4 days of ECMO using, the patient’s lung condition improved significantly. On the 5th day after resuscitation, the patient was weaned off ECMO, extubated, and breathed spontaneously.

At the present, the patient is full alert, stable health condition, continued to follow-up and treatment in Pediatric and Neonatal Surgery Department, Viet Duc University Hospital.

 

Expected to be discharged in the next few days.
