Recently, the Andrology Center of Viet Duc University Hospital received a male patient named L.D.T (12 years old), from Phu Xuyen, Hanoi, due to scrotum pain. Investigating his medical history, the doctors learned that T had scrotum pain for 2 days, but because he was shy, he didn’t tell his parents. It was only when he couldn’t stand it any longer, that he told them and was hospitalized.

At the hospital, he was examined by Dr. Bui Van Quang MSc of the Andrology Center, and indicated an ultrasound for T. He was diagnosed with testicular torsion on the 2nd day. The doctors have performed the operation on emergency. Dr. Bui Van Quang shared: “During the surgery, the entire left testicle was changed to black and purple colors and unable to be preserved. The doctor removed the entire necrotic left testicle, fixed the right testicle to prevent any further torsion that side. After surgery, the patient is stable, continues to be monitored and treated, and expect to be discharged within 1 to 2 days”

According to Assoc. Prof. Nguyen Quang, Director of Andrology Center, Viet Duc University Hospital: Testicular torsion is a common male health in emergency, accounting for about 17% of cases of acute scrotal pain, 90% of it is common in the age of 13 to 21. Testicular torsion occurs when the testicles become too mobile around the spermatic cord, leading to blockage of the vascular system, resulting in acute vascular occlusion, which resolved in time, will lead to testicular necrosis. This is a male emergency requiring urgent surgical treatment especially in the first 6 hours”. Assoc. Prof. Dr. Nguyen Quang also emphasized that the symptoms of testicular  torsion are often: awakened by a sharp pain in the scrotum and the patient can often remember clearly the time of onset. Physical examination can see the torsion side of the testicle was higher than the other side. Doppler ultrasound of the testicular pulse spectrum is valuable for the diagnosis of testicular torsion with typical whirlpool findings. The disease is often misdiagnosed with orchitis, which is the cause of delayed diagnosis and orchiectomy.

Through the above case, the doctors recommend that all men, especially boys have to check in the medical facilities immediately if there are sudden symptoms of pain in the scrotum, and parents should to advise them about abnormalities to detect, diagnose and treat promptly to avoid unfortunate consequences like this case.
