With only 4 steps of electronic medical declaration by scanning QR-code via VIETNAM HEALTH DECLARATION (VHD) application, the declared health data will be updated immediately, saving time and make the medical examination more convenient and faster.

 

 

In order to support people coming for medical examination or treatment to make medical declarations easily and conveniently, Viet Duc University Hospital has applied electronic medical declaration by scanning QR-code via VIETNAM HEALTH DECLARATION application (VHD).

 

Instructions for electronic medical declaration by scanning QR-code:

 

Step 1: On Smartphone, go to Google play app (Android) or App Store (IOS) → Search and install VHD app.

 

Step 2: Sign in with phone number then get the OTP code to access the system.

 

Step 3: Select the function “Domestic health declaration”, update information and click “Send declaration”.

 

Step 4: After completing the information submission, the system will automatically return a “QR-CODE” code to people who can use the “Medical Check-in/Check-out” at the required places.
