Viet Duc University Hospital has received a Japanese patient

On November 1st 2019, Viet Duc University Hospital has received a Japanese patient (65 years old), teacher of  Japanese in Vietnam due to a blunt chest trauma with a left clavicle fracture and dislocation after a motor accident.

 

Assoc. Prof. Dr. Nguyen Huu Uoc, Director of Cardiovascular and Thoracic Surgery Center said: The patient had a medical history at high risk of cardiovascular conditions such as diabetes, gout and a colon surgery for 30 years ago. After being hospitalized, on November 4th, orthopedic surgeons performed a surgery for implant of clavicle  fracture. By the afternoon of the same day, patients suddenly presented hypotension that did not response to medical treatment even with high doses of vasopressors, sweating, left chest pain and multiple ventricular extra-systole in monitor.

 

Combination of clinical examination and results of ECG, cardiac enzymes, Echo Doppler, the patient was diagnosed an acute myocardial infarction with elevated S-T signal. As we all know, acute myocardial infarction is a very dangerous condition that could be life-threatening.

 

Dr. Le Nhat Tien at the Cardiovascular and Thoracic Surgery Center said: for this Japanese patient, a coronary angiography was performed on emergency for diagnosis and an acute blockage of  right coronary artery, 80% stenosis of LC (left circumflex) artery, 50% stenosis of LAD (left anterior descending) artery.  The team of Cardiovascular and Thoracic Surgery Center, Viet Duc University Hospital did the intervention quickly, two stents inserted into right coronary artery stenosis that caused myocardial infarction at night. It took 30 minutes.

After the intervention, the patient was awake, able to self – breathe, hemodynamically stable and the vasopressor was stopped. He no longer felt chest pain and could move normally around. He will be monitored at the Cardiovascular and Thoracic Surgery Center until full recovered.

 

From this case, we recommend that when having risk factors such as: aging, associated with the diseases such as diabetes, hypertension, metabolic disorders…, people should have regular health checks to detect and be treated properly for these dangerous diseases. In addition, it is necessary to have a good screening for cardiovascular diseases before any orthopedic and digestive intervention and surgery.
